The third question is what language is most commonly spoken at home and what type of language if not English and Is your child from an Aboriginal or Torres Strait Islander background? (Example: English, Aboriginal - Example 2 Indian, Hindi).
The fourth question is your full expected costs to participate in activities. (Example: Training fees, exams, uniform, safety equipment, etc)
The fifth question is your child's first and last name and child's Gender (Example: Male-Female-Gender Diverse).
The sixth question is information about the child's parent/guardian. This will require a full name, full address, contact number and email and if your child been identified as living with a disability? (A yes or no question, full details about the child's disability are not required.)
Plus, describe what language is most commonly spoken at home and what type of language if not English and Is your child from an Aboriginal or Torres Strait Islander background? (Example: English, Aboriginal - Example 2 Indian, Hindi).
The seventh question is their age. You can click on the small calendar and navigate to the correct date or simply type it in.
The eighth question is your medicare number and what reference number they appear on it. A VISA number can be used here also if you have no medicare number.
